Integrative Care Assessment

RPS Care uses a team approach with the client, family, and professionals. We empower the client as a partner in the development of a specialized care plan. The plan developed by RPS Care explores the many facets of a client’s life to include; physical, vocational, mental health, spiritual life, and social experiences.

The comprehensive plan we develop includes a multidisciplinary assessment and approach which addresses the client’s weaknesses, breaks down barriers, and builds upon their strengths.

Our goal is to encourage the client to live their best life while pursuing achievable passions in the community.

Treatment

RPS Care will make specific treatment recommendations based upon the results of our integrative assessment. Too frequently clients are given numerous diagnoses without taking into account their social experiences and environmental factors. As a result, they feel “labeled” and disempowered.

RPS Care helps to reduce any diagnostic confusion and create a clear path for treatment whether residential or outpatient services. Both treatment options include our community reintegration guidance and coaching for clients and families.

Advocacy

Following the development of a specific recovery plan for the client and family, RPS Care will then guide, mediate, and intervene when necessary.

At RPS Care, we will facilitate/coordinate the admission process into residential care when such treatment is necessary. We will reevaluate various types of care on an ongoing basis to achieve the best possible outcome for the client.

Guidance

Often, clients are thrown off course by the events in their lives. For mid-age or older adults, the impact of these events can be very different than in previous years.

At RPS Care, we help to support the client and family throughout their recovery journey. If they need to change course or add a professional to their care team, we provide guidance and coaching in the decision-making process.

RPS Care management services aids in reducing the client’s stress of community reintegration and post-treatment recovery, while at the same time alleviating the emotional burden on the family. Our approach allows the family and significant others to have the necessary time and energy to rebuild their relationships.

How Long Will Our Care Management Service Last?

The length of time of our care management engagement will vary based on the acuity level of the case and needs of the client.

Most care management services last for twelve (12) months following discharge from residential care or ongoing participation in our outpatient community care plans.

Progress Tracking

We provide periodic performance tracking for clients/families for a period of one to three years as the situation may warrant. We will guide, mediate, or intervene as the needs of the clients/family change. We measure outcomes based on a variety of tools to include periodic reports which incorporate the measurement of variables and key indicators for a sustained recovery.
